Quality Outreach RN – Care Coordinator We are seeking an experienced and motivated Quality Outreach RN to support quality outreach initiatives and member engagement efforts. This contract opportunity is ideal for nurses with strong HEDIS, quality improvement, and managed care experience who thrive in a remote, performance-driven environment. The Quality Outreach RN will leverage clinical knowledge and quality program expertise to conduct member outreach, support care gap closure, and contribute to improved quality performance and member outcomes.

Key Responsibilities

  • Conduct outbound member outreach to support HEDIS gap closure and preventive care compliance
  • Coordinate care across providers and healthcare settings to support continuity of care
  • Monitor member progress and identify barriers to treatment adherence
  • Review medical records, claims data, and HEDIS measures to identify intervention opportunities
  • Support quality initiatives focused on improving HEDIS, Stars, and member experience metrics
  • Document outreach activities, barriers, and outcomes accurately and in a timely manner
  • Collaborate with clinical and quality teams to support reporting and compliance initiatives
  • Maintain HIPAA compliance and adhere to all regulatory requirements

Qualifications

  • Active, unrestricted RN license (compact license preferred)
  • Required HEDIS experience, including gap closure and quality outreach work
  • Experience with Medicare, Medicaid, or other government programs populations strongly preferred
  • Minimum 3–5 years of clinical experience in managed care, population health, or quality programs
  • Strong communication, organization, and documentation skills
  • Experience working in remote or telehealth environments preferred
  • Proficiency with EMR systems and quality documentation platforms

Contract Details

  • Fully remote contract role
  • Contract Duration: 6 months
  • Standard Monday–Friday schedule EST

Compensation:

  • $45/hr – 1099 (LLC rate)
  • W2 option may be available at an adjusted hourly rate
