JOB DESCRIPTION AND POSITION REQUIREMENTS Scheduled Weekly Hours: TBDDay shift - weekday and weekend shifts JOB SUMMARY: Care Coordination is a collaborative process in which the Care Coordinator assesses, plans, implements, coordinates, and evaluates options and services to meet an individual’s health needs. The Care Coordinator facilitates progression of care and monitors the patient's progress to ensure that the plan of care and services provided are patient focused, high quality, evidence based, appropriate to patient needs, efficient, and cost effective.
